Evidence-Based Program: ¡Sí, Yo Puedo Controlar Mí Diabetes! (¡Sí, Yo Puedo!)

¡Sí, Yo Puedo!  is a culturally tailored 4-week diabetes self-management education program. Classes are conducted weekly and are approximately two hours in length. Each lesson module is made up of several educational activities to reinforce diabetes self-care behaviors. ¡Sí, Yo Puedo! utilizes the social cognitive theory and the self-regulation model as its theoretical foundation to promote behavior change.

  • Target audience: Tailored for Spanish-speaking, lower-literate Hispanic/Latinos adult over the age of 18 with self-reported diagnosis of type 2 diabetes (T2D).  Program also appropriate for bilingual speaking Hispanic/Latinos.
  • Health outcomes:
    • Improved diabetes self-care skills
    • Enhanced diabetes self-efficacy
    • Increased diabetes-related knowledge
  • Delivered by: 1 facilitator, who can include trained health professionals, County Extension Agents (CEA), Certified Diabetes Educators (CDE), and/or Certified Community Health Workers (CHW)
  • Program type: Group
  • Format: In-person in community and remote.

Note:  It is permissable to use a combination of remote (i.e.videoconference) and in-person implementation in the same session or during a class series. For example: 10 participants join by Zoom and 10 participants attend in a senior center and receive instruction at the same time.
