Evidence-Based Program: TCARE® Support System (Tailored Caregiver Assessment and Referral®)

The TCARE® (Tailored Caregiver Assessment & Referral) evidence-based system uses intelligent, predictive technology to precisely target & map interventions, reducing family caregivers’ burden & stress levels. Care professionals use TCARE’s web-based system to assess a family caregiver’s social determinants of health, identify specific risk factors related to stress and depression & create individualized, tailored care plans that help support self-care, drive down the risk of burnout, and connect to services that enhance the caregiver's role. By surfacing the right interventions, at the right time, TCARE helps every family caregiver better navigate their caregiving journey and supports programs by ensuring the delivery of resources such as training, respite, in-home service will get to those at greatest risk first.  TCARE leverages machine learning and Artificial Intelligence technology to:

  1. Assess the unpaid & paid family caregiver,
  2. Determine risk/stress points,
  3. Link to local community resource, and
  4. Help caregivers understand their role and the affects that caregiving has on their own well-being.

Trained and Certified TCARE Specialist utilize the outcomes of the assessment to facilitate this process. Standardized reporting of outcomes and workflow support are also part of the system.

  • Target audience: TCARE partners with Managed Care Organizations, Managed Long-Term Services and Supports (MLTSS) Plans, State Units on Aging, Area Agencies on Aging, Long Term Care insurance carriers, and health systems. Older American's Act National Family Caregiver Support Program (Title III-E) and Older Americans Act Health Promotion Programs (Title III-D).
