Create or Modify a National CDSME Database Account
Complete the Grantee and Users Intake Form to create your account in the National CDSME Database and Power BI. This information will help us create your organization’s profile and network page, and set appropriate permissions for database users.
- The number of users for the National CDSME Database is limited to 5, with one assigned to use PowerBI.
- Follow the instructions on the intake form to indicate whether each individual will serve as a “lead contact” or “general contact” affiliated with your network.
- Also indicate which database users will have “full administrative access,” and who will have “data entry only” access levels.
