What Is a Part D Indication-Based Formulary?

Beginning in 2020, Medicare drug plans have the option to utilize an indication-based formulary. This means that Part D plans can choose to cover a drug for specific uses/conditions, and not all of the uses approved by the Food and Drug Administration.
